Receptionist

RECEPTIONIST
Hospice of CNY has a need for a receptionist. This person is the first line of communications to establish and maintain a company-wide communications infrastructure.
The hours are Monday-Friday from 8:00am-4:30pm.

Job Description

Qualifications and Skills – Receptionist:
Must have superior oral communication skills and be compassionate and efficient

Education: High School Diploma or equivalent

Experience: Experience in handling multiple telephone lines

Credentialing/Licensing: None

Responsibilities – Receptionist:

  • Responsible for receiving all incoming phone calls, vendors, and guests and ensuring they are disseminated or directed to the appropriate location in accordance with Hospice procedures
  • Responsible to provide administrative assistance as requested (sorts incoming mail and distributes, assists other departments with projects, collects, creates, and coordinates conference room reservations in Microsoft Outlook, assists with general clerical duties as needed).

Development Coordinator

Job Description – Development Coordinator 

Hospice of Central New York and The Finger Lakes is hiring a Development Coordinator to join our development team!

Position Summary: This position supports the Foundation in all aspects of fundraising including but not limited to events coordination, the cultivation of donor solicitation and sustaining donor relations, the oversight of social media management, and any other similar related duties.

Minimum Education: Bachelors Degree or related equivalent experience

Minimum Experience: At least two years of paid fundraising experience and event coordination; Experience with Raiser’s Edge & Canva is preferred but not required.

Qualifications:

Job Duties:
Responsible for the oversight and execution of Development fundraising events:
· Plans and coordinates the details of each fundraising event throughout the year
· Solicits corporate sponsorship and in-kind support for fundraising events and activities
· Works with event committees to support their efforts of soliciting corporate support and in-kind donations
· Develop new events and fundraising activities to stay current and connected with the community
· Attends Hospice community events & Foundation fundraising events, as needed
· Provides data and feedback of events for potential future improvements

Provides oversight and management of media and marketing relating to fundraising:
· Designs, develops, coordinates, and creates any marketing materials for events and any other Foundation related activities or needs
· Provides maintenance and update fundraising activities on the public Hospice website
· Posts in and manages content listed on the Hospice social media platforms

Provides a variety of administrative support to the Development Team and to Hospice:
· Research and write grant opportunities that support Hospice or as requested
· Serves as back-up to Development Database Coordinator
· Provides data and feedback relating to revenues and expenses relating to annual budget
· Generate invoices for event sponsors and attendees, as needed
· Monitor and track sponsor and attendee payments

Hospice Admissions Social Worker

Job Description – Hospice Admissions Social Worker

Hospice of Central New York and The Finger Lakes is hiring Social Workers to join our team!

Social Workers are part of a dedicated, compassionate, Hospice interdisciplinary team. They play a vital role in helping patients and caregivers with social, emotional, and practical challenges through the end-of-life journey.

Alongside a registered nurse, Admission Social Workers will:
– Provide education about Hospice philosophy and end of life, assess for safe caregiving plans, complete psychosocial assessments and collaborate with patient and family to develop an individual and unique plan of care.
– Offer patients and families support and interventions regarding anticipatory grief and the end-of-life process.
– Provide community resources, education regarding Advanced Directives and offer expertise in problem solving and coping, involving caregiving and decision making.
– Complete all required documentation, collaborate with primary Hospice teams for continuation of care and participate in organization wide education.

Hospice care primarily takes place within a patient’s home but may include residential settings such as comfort care residences or group homes. Hospice of Central New York and The Finger Lakes serve patients within four counties in Central New York (Onondaga, Cayuga, Oswego and Madison)

Weekend Triage Registered Nurse

Weekend Triage Registered Nurse

Job Description:
The Weekend Triage Registered Nurse is responsible for the oversight of all inbound phone inquiries relating to patient care, and assesses and troubleshoots received inquiries to provide resolutions, if possible. Provides nursing care to patients and families in accordance with the policies and procedures set by Hospice and governing bodies. The Weekend Triage RN demonstrates a high standard of nursing practice related to end-of-life care and pain and symptom management through competency evaluations. 

Qualifications and Skills – Weekend Triage Registered Nurse must have:

  • Education: Must be a graduate from an accredited school of nursing.  BSN degree preferred, but not required.
  • Experience: Must have a minimum of two year’s work experience as a professional nurse within the past five years in a hospital, home care, nursing home, oncology or medical/surgical setting
  • Credentialing/Licensing: Must possess current professional license as a Registered Nurse with New York State and be in good standing.  Proof of license will be required per New York State Department of Health

Responsibilities – Weekend Triage Registered Nurse:

  • Responsible for the delivery of patient care through the fundamental nursing process of assessment, planning, implementation and evaluation within established guidelines
  • Provides phone triage and support to patients/families from 6:30am to 6:30pm each Saturday and Sunday
  • Maintains current patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur
  • Documentation of calls and concise, accurate, timely follow-up
  • Provides appropriate support at the time of death
  • Operates as a resource by providing advice, opinions and counsel utilizing problem solving skills to patients & families, and to other personnel
  • Maintains regular communications within all Interdisciplinary Groups (IDGs), as appropriate
  • Initiates communication with attending physicians, Hospice Medical Director, other hospice staff members, and other agencies to coordinate optimal care and use of resources for the patient/family, as needed
  • Provides coverage as the Administrator on call (support on-call RN providing triage phone role, provides referral and informational calls as needed, dispatches nurses, social workers and chaplains as needed)

Job Type:     Saturdays and Sundays from 6:30am to 6:30pm.

Salary Range:  $38.00 – $43.00 per hour

REGISTERED NURSE CASE MANAGER

Our Admissions Department is seeking Registered Nurses (RNs) to serve as the first point of contact for patients as they enter our program. On-call Spiritual Care/Chaplain

ON-CALL SPIRITUAL CARE/CHAPLAIN

Job description

Hospice of CNY is a growing agency seeking an On-Call Spiritual Care/Chaplain. The On-Call Chaplain provides spiritual care to patients & families in a variety of settings in accordance with the plan of care. The On-Call Chaplain is responsible for the delivery of care through the application of advanced knowledge of concepts, practices and procedures:

  • Completes psychosocial/spiritual care assessments as directed
  • Provides direct service spiritual care/spiritual counseling to patients and families
  • Must have the ability to respond on short notice at any hour to diverse & unique requests for spiritual care. This response may require telephone contact or direct visitation.
  • Accept responsibility for specific coverage times on a monthly basis according to need and
  • Provides all documentation via mail, fax or in person by Monday morning after the weekend worked on-call.

Qualifications:

  • Affiliation with a recognized faith group
  • Specialized training in clinical chaplaincy or spiritual psychotherapy
  • Ability to respect the dignity of all who serve and are served
  • Ability to relate to diverse faith groups with openness and respect
