Our specially trained team of health care professionals can provide care and support in your home.  The team is made up of physicians, nurses, social workers, chaplains, Home Health Aides, and volunteers whose goal is managing your unique symptoms and enhancing your emotional and spiritual well – being while remaining in your home.  This team is available 24 hours a day to respond to your needs and will make home visits when needed. If your symptoms are not able to be managed at home, we will help arrange for hospitalization and work with the hospital staff to get your symptoms under control and get you home as soon as possible. Our services are also available in nursing homes and assisted living centers. We can go to where the patient is best served.

Each person is different and the way the disease affects them will also be unique. Hospice care may be right for you depending on how the disease is progressing. The best way to determine this is with a health assessment. Call us at 315-634-1100 and set up an assessment with our admissions team. It costs nothing and it will answer your concerns about Hospice care.
