CCH Home Care & Palliative Services is a certified home health agency reaching out to patients throughout Onondaga County. And, because every patient has their own unique set of health care needs, our programming is specifically designed to meet their changing needs. All care is safely delivered under the supervision of the patient's physician. What's more is that there are reimbursements available from Medicare, Medicaid, and private insurance that can help cover the cost of care.

What Services Do We Provide?

Disease Management & Education

Provides care and support for ongoing illness. We help enhance the ability of our patients and their loved ones to manage on ongoing illness. This happens through education as well as coordination of community resources and care providers.

Post-Acute Care

Assists patients in returning to the best quality of life after hospitalization. The home care team renders the direct care and educational services essential to easing the return to health and independence.

Rehabilitation

Designed to help patients regain function and activity. Physical, speech, and occupational therapists provide direct therapy and education to assist the patient in achieving their maximum level of independence.

Palliative Care

The word "palliative" is complex, but its meaning is quite basic--palliative focuses on comfort and quality of life. This program is for the active and total care of patients whose disease is progressive. Emphasis is placed on the control of pain and other symptoms, while also providing psychological, social, and spiritual support.

Depending on the patient's needs, the home care team is comprised of one or more of the following:
